Not exactly from an external viewpoint, but I try not to think in terms of words. If I just spat out everything I had in my head, I would be distracted from my goal. So instead I start with the idea, and go to words from there.
I think when people try to just copy the inside of their head, it's like translating a translation. There's too many layers of complexity. (Of course, copying one's internal dialogue is much better than trying to go through even more complicated procedures, which is how a lot of writers can get in trouble and feel exhausted.)
